A new CMS demonstration program opens GLP-1 access to Medicare patients on July 1, 2026. Here is what it covers, who qualifies, and where to get the documentation details.
What the Program Covers
A short-term Centers for Medicare & Medicaid Services (CMS) demonstration, not a permanent Part D benefit change.
The Medicare GLP-1 Bridge sits outside the standard Part D benefit structure. Part D plans do not need to opt in, and the $50 copay does not count toward the deductible or true out-of-pocket costs.
The program covers weight management only. Patients using a GLP-1 for type 2 diabetes, moderate to severe obstructive sleep apnea, or MASH stay covered through their existing Part D plan and do not qualify under the Bridge. Any physician who prescribes GLP-1s or manages Medicare patients with obesity or related cardiometabolic risk will want to understand how this program works.
Who Qualifies
A patient must meet one of three thresholds, attested at the time GLP-1 therapy started.
BMI ≥ 35
No additional diagnosis required.
BMI ≥ 30
With heart failure with preserved ejection fraction, uncontrolled hypertension, or chronic kidney disease stage 3a or higher.
BMI ≥ 27
With pre-diabetes, a prior heart attack or stroke, or symptomatic peripheral artery disease.
Patients must be at least 18 years old and using the drug alongside ongoing lifestyle modification.
Prior authorization and prescriber support
Prescribers do not need to be Medicare-enrolled to write the prescription or submit the prior authorization, though they cannot appear on CMS's Preclusion List. CMS is using a single central processor in 2026 to manage prior authorization, claims, and pharmacy payment. Requests can go in electronically or by fax, and CMS is not accepting them before July 1.
